If you have Microsoft Word and Excel you can open the documents below
on your computer, complete the forms, print them, and mail or deliver
them to the SCORE office. You can save a copy of them as Templates
(dot and xlt) and access them for New Office Documents as you do other
Templates. This means you would not have to be connected to the
Internet to use them.
The 641, 641A, and Travel Voucher in Word and Excel formats
were provided to us by the Annapolis SCORE Chapter. We appreciate their
generosity.
If you do not have Word or Excel on your computer, you can download a viewer from Microsoft's website which will allow you to print the forms and THEN complete them by hand. The current link for the Viewers is shown below. Since websites change over time, the location on Microsoft's site may change. If that happens, go to www. microsoft.com and search for Viewers.
You can also open the PDF versions using your Acrobat Reader, print the forms, and THEN complete by hand.
